To request a free pickup please fill out the following form or call your United Visual Representative or call Ashley Carlson at 630-467-1448.  If you phone for a pickup, please be sure to specify you want summer service. (All information submitted to United Visual will be held in confidence.)

Please keep in mind this is a request and is subject to change.  Regardless of when your equipment is picked up we guarantee delivery of your completed equipment before the school year resumes.

Please estimate the amount as accuratley as possible. If you do not have a fairly accurate estimate at this time, please note it above stating "rough" estimate.


Estimates:
  I approve all overheads requiring additional work up to $75
    or without an estimate.
  I approve all VCRs requiring additional work up to $65
    or without an estimate.

Please mark all units being sent in as "Summer Maintenance" or "SM".  Please remember to remove all overhead roll attachments, acetate rolls, and cores whenever possibile to eliminate any additional charges.  If you are aware that a unit needs additional work, please mark it accordingly.
